REGISTRATION

Registration fees are covered by educational grants. The fellow must be in their Interventional or General Cardiology fellowship to qualify for this symposium.

Please note: Space for this meeting is extremely limited; therefore, registrations are accepted on a first-come, first-served basis. Upon registering, attendees must submit a letter from their program director stating that they meet the qualifications for this symposium; this letter must include the following:

  • Fellow's full name
  • Fellow's email address
  • Fellow's phone number

Registration is not confirmed until the applicant receives the final confirmation email from Moceri Management.

CASE SUBMISSIONS

At Piedmont Heart Institute's Cardiovascular Fellowship Series 2011, Sunday's program will focus on a Best Case and Worst Case Cine Presentation competition of cases submitted by attendees. Fellows are requested to send a best and worst interventional case in advance of the meeting. If a fellow's submission is selected, he or she will be asked to present it at this session. Winners will be announced at the end of the session and prizes will be awarded.

Please note the following:

  • All cases must be submitted in Microsoft PowerPoint (.ppt) format
  • Please submit all cases to Melissa Vann (mvann@MoceriMgmt.com) no later than Monday, April 18, 2011
  • If a presentation contains large embedded video files, please upload these to www.YouSendIt.com and insert mvann@MoceriMgmt.com or submit cases on a CD-ROM by mail to Melissa Vann at Moceri Management, 1100 Johnson Ferry Road, Suite 75, Atlanta, GA 30342
  • Due to HIPAA guidelines, at no time during the case review can the patient name or identification number appear on the projection screen or be disclosed to any of the course participants. Please remove patients' names from all materials.
